Core Registry
To add patients to the Core Registry, informed consent is required. The table below contains information sheets and consent forms in use in the centres listed. These information sheets and consent forms have been created on the template from the paperwork in English created in the UK where the original ethics and data governance approvals were obtained in 2019.
